What the joint actually does
The jaw joint does two things at once. It rotates like a hinge for the first inch of opening, then slides forward down a slope in the skull for the rest of the range. Between the ball and socket sits a small disc that moves with the jaw and cushions the load.
You have two of these joints joined by one rigid bone, so neither moves without the other. That is why a problem on one side produces symptoms on both, and why the muscles running into the temple and down the neck end up involved in nearly every case.
The disc is where the noises come from. If it slips forward of the ball and is recaptured as you open, you get a click. If it stays out of place, the jaw may not open as far. Clicking on its own, with no pain and no limit on opening, is common and often left alone.
Symptoms that bring people in
Jaw problems refer pain widely, which is why many people arrive after seeing someone else first.
- Morning headaches. A dull ache in the temples on waking, sometimes a band around the head. It comes from muscles that worked all night.
- Jaw fatigue and soreness. Aching in front of the ear or along the angle of the jaw, worse after chewing steak or bagels.
- Clicking, popping or grating. Noise on opening or closing. Grating tends to mean more than clicking does.
- Locking. A jaw that will not open past two fingers wide, or one that opens and will not close. This one needs seeing sooner rather than later.
- Ear symptoms with a normal ear exam. Pain, fullness or ringing when the doctor found nothing wrong with the ear. The joint sits against it.
- Worn, cracked or sensitive teeth. Flat shiny facets, chipped edges, cracked fillings, teeth that ache in the morning. Teeth grinding is often noticed at an exam first.
Most of it is muscle, not joint damage
Most of the jaw pain we see is muscular. Clenching and grinding load the chewing muscles for hours, they become tender and shortened like any overworked muscle, and the pain spreads into the temple, the ear and the neck. The joint itself is often fine.
Grinding mostly happens during sleep and is not under your control. Daytime clenching is a habit tied to stress or concentration, and most people have no idea they do it until someone asks. Gum chewing, nail biting, chewing on one side, an old injury and whiplash all contribute. Arthritis and damage inside the joint account for a smaller share.
Dental causes exist and are less common than the internet suggests. A filling or crown left slightly high, a tooth lost and never replaced so the bite collapsed on that side, or contacts that make the jaw slide sideways to close can all keep muscles working. How we evaluate it
The examination is mostly hands and questions. This is not a scan-first problem.

History
When it started, what makes it worse, whether it wakes you, previous injuries, and what you have tried. Pain worst in the morning and pain worst in the evening point in different directions.

Palpation
We press the muscles in the cheek and temple, the muscles under the jaw, and the joint itself, and ask what reproduces your pain. Finding the exact spot tells us more than imaging does.

Range of motion
How far you open, measured in millimeters, whether the jaw deviates to one side on the way, and where in the arc a noise happens. Repeat measurements show whether anything is improving.

The bite and the teeth
Wear facets, cracks, loose teeth, high restorations and missing teeth, and whether the teeth meet evenly across both sides.

Ruling other things out
X-rays when the history suggests joint change, an abscessed tooth or a fracture. Some jaw pain is not the joint: dental infection, sinus problems and nerve pain all mimic it. Anything that does not fit gets referred.
Conservative treatment, including a night guard
Most cases settle with measures that change nothing irreversibly. They work better combined than tried one at a time.
- Rest the jaw, with heat or cold
- Soft food for a couple of weeks, cut small. No gum, no chewy bread, no ice, and support your chin when you yawn. Moist heat suits muscular pain and cold suits an acutely inflamed joint.
- Habit awareness
- Teeth apart, lips together. Teeth should only touch when you are eating. Symptoms track stressful stretches, and most daytime clenchers break the habit within weeks once they notice it.
- Physical therapy
- Stretching, posture work and manual therapy for the jaw and neck. For stubborn muscular cases this often helps most, and we will refer you.
- A night guard
- A custom hard guard worn while you sleep. It protects teeth from grinding and lets the muscles settle. It does not stop grinding, a soft drugstore tray can make clenching worse, and it costs less than replacing worn teeth with crowns.
- The obvious dental contributors
- Adjusting a filling or crown that sits high, restoring a broken-down tooth, or replacing a missing tooth so the bite is supported. Small and specific, not a rebuild.
Questions about jaw pain
Will my jaw clicking get worse?
Often it does not. Clicking that does not hurt, does not limit opening and has been stable for years is usually watched rather than treated. New or painful clicking needs looking at.
Does TMJ go away on its own?
Many episodes settle within weeks, especially after a stressful stretch or a long dental appointment. Symptoms lasting beyond a few weeks, or returning repeatedly, are worth evaluating.
Do I need an MRI?
Rarely. Imaging helps when the history points to damage inside the joint, when a lock does not resolve, or when conservative treatment fails. Most muscular jaw pain is diagnosed by examination.
Can a night guard from the pharmacy work?
It might get you through a bad week. Long term a boil-and-bite tray fits loosely, can move teeth and is often too soft to do the job.
Is my jaw pain caused by a bad bite?
Sometimes a high filling or a collapsed bite contributes, which is why the bite is examined. More often it is a passenger rather than the driver, and that decides whether treatment is small or invasive.
