Forty years is a long time to watch teeth

Dentistry graduating classes in 1981 learned to read film, mix amalgam by hand and take impressions in a tray full of material that set in your mouth. Since then the imaging went digital, the bonding chemistry that holds tooth colored restorations in place became genuinely reliable, implants moved from unusual to routine, and the understanding of gum disease changed from a plaque problem to an infection with consequences beyond the mouth.

Keeping up with that is the job, and it is why continuing education is a condition of holding a California license rather than a nice extra. The other half of the job is knowing which new thing is worth adopting and which is a product launch. Long practice is mostly useful for that second part.

The unglamorous advantage of one dentist staying in one place is the timeline. A crack that appeared between two visits, a filling that has quietly gone soft at its edge, gum recession that moved a millimeter in three years: none of that shows up in a single appointment. It shows up in a record kept by the same person, compared against images taken years apart.

A general dentist, and why he says so plainly

California is strict about how dentists describe themselves, and for good reason. A patient should be able to tell from a web page whether the person they are booking with is a general dentist or a specialist who completed several additional years of accredited postgraduate training in one field. Dr. Brosi is a general dentist.

In practice that covers most of what walks through the door. Exams, cleanings, fillings, crowns, bridges, dentures, root canal therapy, non surgical gum treatment, implant restoration, clear aligners, night guards, sleep appliances and emergencies. See everything the practice does.

Where a case is better handled elsewhere, he will say so and help you get there. That includes complex surgical cases, some difficult root canal work, unusual periodontal cases and children who need care beyond what a general office should be doing. Nobody benefits from a dentist working at the far edge of their own comfort.

Why a small practice changes the work

A town this size has consequences for a dentist. You run into your patients at the hardware store. Word about how you handled somebody who could not pay travels faster than any advertising, and it travels in both directions. Most new patients here arrive because someone who already comes in told them to, which is the only form of marketing that has ever really worked.

It also changes the arithmetic on treatment. A dentist who will see the same patient for twenty years has no incentive to do work that fails in five. The conservative option is not just a philosophy here, it is the option you have to live with in front of the person you did it to.

The practical version of that is continuity. The same dentist has looked at your x-rays every year, so a shadow that has changed slightly gets noticed. A crack recorded three years ago gets checked rather than rediscovered. That is worth more than any single piece of equipment in the building.
