Living at Elevation: Dry Mouth, Sports Drinks, and Sierra Teeth
Well water, dry air, long days outdoors, and a bottle that never empties. How foothill and mountain habits show up in teeth, and what to do differently.

Oakhurst sits in the Sierra Nevada foothills at the southern gateway to Yosemite, and the way people live up here shapes what we see in the operatory. Not because mountain air does something exotic to enamel, but because of ordinary habits: what comes out of the tap, how long a drink lasts on a trail, and how dry a house gets with a wood stove running.
This is general education about patterns, not a diagnosis. Nothing here can tell you what is happening in your own mouth, which takes an examination and usually x-rays.
Well water and the fluoride question
Plenty of households in the foothills draw from a private well or a small local water system rather than a large municipal supply. That matters for teeth because fluoride occurs naturally in groundwater at levels that vary from one well to the next. Some wells contain very little. Some contain more than a public system would deliver on purpose. There is no way to guess from the taste, the hardness, or the neighbor’s well.
The only way to know is a test. Have your water analyzed by a certified laboratory and ask specifically for fluoride, in addition to whatever else you are testing for. Keep the report. Then bring the number to your dentist or your child’s physician.
That number matters most for children whose permanent teeth are still forming. Fluoride supplements are prescribed based on the actual concentration in the water a child drinks daily, the child’s age, and what other sources are in play. Both too little and too much are real considerations, which is why supplements are a prescription decision rather than something to add on a hunch. Adults benefit from fluoride too, but through toothpaste and in office applications rather than by swallowing it.
Two footnotes worth knowing. Most bottled water contains little or no fluoride unless the label states otherwise, so a household that drinks exclusively bottled water is getting close to none regardless of what the well contains. And most home filters do not remove fluoride, although reverse osmosis systems and distillation do. If your kitchen has a reverse osmosis tap, mention it at your next visit.
Why dry mouth is the quiet problem
Saliva is doing more work than most people realize. It rinses food off the teeth, it neutralizes acid after meals, it carries calcium and phosphate that help repair the earliest stages of damage, and it holds antibacterial proteins. When saliva drops, all of that drops with it, and the risk of decay climbs quickly.
Dry mouth up here usually has ordinary causes. Wood heat and forced air heat pull moisture out of indoor air all winter. Allergy season, a deviated septum, or a stuffy nose leads to breathing through the mouth overnight, and people wake with a mouth like paper. Many common medications list dry mouth as a side effect, including a number of blood pressure medications, antidepressants, antihistamines, and bladder medications, and the effect compounds when someone takes several. Alcohol and heavy caffeine contribute. So does snoring.
None of that is about altitude, and you should be skeptical of anyone who tells you the elevation itself is drying out your teeth. What is true is that the combination of dry indoor air, allergy medication, and a lot of time outdoors puts a good number of people here in a low saliva state for hours at a stretch.
Things that help: a humidifier in the bedroom during heating season, water at the bedside, treating the nasal congestion that is causing the mouth breathing, sugar free gum after meals to stimulate flow, and a review of your medication list at your next appointment. Over the counter dry mouth rinses and gels help some people. If your mouth is persistently dry, say so, because a high fluoride toothpaste or more frequent cleanings may be reasonable, and that is a conversation to have during an exam and cleaning.
The bottle that never empties
Here is the mechanism that explains most of what we see in people who spend their days outside.
Every time you drink something sweetened or acidic, the surface of your teeth becomes acidic for a while afterward, then saliva slowly brings it back. Teeth handle that recovery cycle well when it happens a few times a day. What they handle badly is a cycle that never completes. A sports drink sipped over three hours on a trail keeps the mouth acidic for most of those three hours. The same bottle drunk in ten minutes at the trailhead does far less. The total sugar is identical. The exposure time is not.
That pattern shows up everywhere in foothill life: the energy drink in the truck cup holder on a long day, the sweet iced coffee that lasts all morning, the soda cooler on the boat at Bass Lake, the water bottle refilled with electrolyte mix on a hot afternoon.
Sports drinks and energy drinks are typically acidic in their own right, separate from their sugar, and so are citrus flavored waters, kombucha, and sparkling waters with added flavoring. Acid alone can wear enamel without any help from bacteria. That is erosion, and it looks different from a cavity: the biting edges of front teeth go translucent and thin, cusps flatten into shallow cups, old fillings start to stand slightly proud of the tooth around them, and everything becomes sensitive to cold. Decay is a bacterial process that drills a hole in one spot. Erosion is a chemical process that thins whole surfaces. Some patients arrive with both.
Practical changes that are actually achievable
- Make plain water the default for hydration. Save the electrolyte drink for when you are genuinely working hard, and drink it in a sitting rather than nursing it.
- Rinse with plain water after anything sweet or acidic. It costs nothing and it shortens the acid exposure.
- Do not brush immediately after acidic drinks or after vomiting. Enamel is softened right afterward, and it is more cautious to rinse, wait a while, then brush.
- Watch the sticky trail foods. Energy gels, chews, granola bars, and dried fruit pack into the grooves of molars and stay there. If you rely on them, carry a small brush or at minimum rinse hard.
- Chew sugar free gum after eating when brushing is not an option. It works by stimulating saliva flow, which is exactly the thing that is missing.
- If you have gum recession, pay attention to the exposed root surfaces. Root surface is softer than enamel and gives way faster, and it is a common site for decay in older adults.

Distance is part of the calculation
Prevention is worth more when help is further away. A broken tooth on a Friday evening in Wawona or North Fork is a different problem than it is in a city, and Fridays through Sundays the office is closed. It is a reason to change what is in the bottle and how long it takes to finish it, and to have someone look at the wear patterns on your teeth before they become sensitive.
